Your first visit to Southview Dermatology involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You’ll find all the practical information you need, such as a map and directions to our office, policies and more.

Arrival

Please plan to arrive 15 minutes before your appointment to fill out the appropriate paperwork.

Insurance

Please bring with you a copy of your insurance card and photo identification to your next appointment.

Please remember that your health insurance is a contract between you, your employer, and your insurance carrier. Your insurance plan determines the amount of your co-pay and deductible. You are responsible for paying this amount. Co-pays are due at the time of your visit. If you cannot pay your co-pay we will ask that you reschedule the appointment.

Please contact your insurance company if you are unsure of your benefits.

Referrals

Any patient who comes in for an appointment without their referral (if their insurance company requires one) will have to pay for that visit or reschedule at a later date.

Payment

Our practice accepts cash, personal check, Visa, Master Card, American Express, CareCredit, and money orders.

Some elective procedures may require partial or full prepayment before they can be scheduled and those appointments will be canceled if arrangements are not met.

Some diagnoses might be considered cosmetic and not covered by your insurance company. We will take payment on the date of service. If you have any questions regarding your coverage, please contact your insurance company.

Underage Patients

Children and young adults under 18 will need to be accompanied by a parent or legal guardian. Our providers are not able to see small children without a parent or legal guardian present. Once the initial visit has been completed, young adults of driving age can be seen without a parent or legal guardian for routine checkups.

Financial Responsibility

If there is a question of financial responsibility, the parent or legal guardian that accompanies a minor to see a provider will be responsible for the account.

Missed Appointments

Please call the office 205.918.1475 if you know you are going to miss an appointment. This would allow another patient to be seen in your place.

Uninsured Patients

Unfortunately, in keeping with the guidelines set forth by Southview Medical Group, we are unable to service uninsured patients who do not first meet the requirements of and register with the Indigent Clinic at St. Vincent’s Hospital.

Workers Compensation

Patients must bring written authorization with their claim number, company name and mailing address plus the adjusters name, mailing address and telephone number.

Prescription Refills

When you request a prescription refill, there are several internal processes that must take place before the provider is able to review your prescriptions. We process those requests as soon as possible, but in some circumstances, this process may take up to 2 business days. Therefore, be sure to take this into consideration when requesting refills.

Telephone Calls

All telephone calls from our patients are important to us and we want to respond to you as quickly as possible. However, some calls might need to be reviewed and discussed with Dr. Cash before a return call to you can be made. Please take this into consideration when calling our office with questions.

Emergencies

A provider is always available to handle any emergency. However, if you cannot reach one please proceed to an immediate care center. Routine medical questions are best handled during office hours when your entire medical record is available to your provider.
